2. Call our office to schedule an appointment: (480) 890-9000

Documents to have at hand before placing the call:
- Insurance card with ID number and contact phone number (on back of card)
- Prescription and name of your physician
- The condition requiring treatment

3. Print our intake forms and fill them previous to your visit. This can save you 20 - 30 minutes. We can also fax these documents to you. Please request a fax at (480) 890-9000
